3 - Child Protection

Assessor Introduction Package - The Duke of Edinburgh's Award (NSW Division) - this package incorporates the NSW Dukes-specific Volunteer Declaration (see pages 7 and 8). This Volunteer Declaration replaces the previous NSW Student/Volunteer Declaration for Dukes NSW purposes. If you have any questions regarding completing this form please contact Working With Children Customer Service on (02) 9286 7276.

The Package also incorporates the National Volunteer Code of Conduct (see pages 5 and 6, and the Agreement on page 9)
